Outreach Services – Long Term Health Conditions and Vulnerable Persons Project

The project is designed to provide additional support to those who need it most by providing weekly outreach clinics at the Carers Centre, Homestart, Reachout, Hawkhill and also drop in services in Alva and Tullibody. We also provide home visits to people with long term health conditions and people with particular vulnerabilities.  

We provide tailored one to one advice to clients on income maximisation, benefit entitlement and dealing with money issues and help those who are in need of support to deal with the complexities of benefits system.

The project takes referrals from other agencies and in main we get most of our referrals from the Carers Centre, Hawkhill Community Centre, Reachout, Integrated Mental Health and Homestart.

We also highlight social policy issues relating to health and vulnerability and provide guidance to support staff from our partner agencies.

The projects are funded by the Alliance and Scottish Government.

Home visits are currently suspended for the time being and outreach services are restricted to telephone and email. We are providing support on benefits issues for our partner agencies staff during COVID restrictions and we will be providing training to staff on sickness and disability benefits.

We are currently looking at how we can restart our outreach services in Alva and Tullibody which have been suspended due to the venues being closed and will provide an update on the opening of our outreach services when this is finalised.

An updated timetable for outreach services will be published when face to face services resume.
